With a second opinion, individuals will benefit from a face-to-face meeting with a MALS specialist to discuss their history and any prior diagnoses. To speed up the appointment process, please gather the following information:
Below are the medical records that we typically request from our patients when evaluating for MALS compression. The most important information needed first is a CD with CT scan and mesenteric duplex. A mesenteric duplex is a very beneficial diagnostic test in determining blood flow through the vessel to assist in confirming a MALS diagnosis.
1. Duplex images on CD along with paper report
2. CT scan or MRA images on CD along with report
3. Angiogram images on CD along with report
4. Consultation reports from Gastroenterology
5. Test reports for colonoscopy, upper endoscopy testing.
6. Consultation and last office note from pain management
7. Consultation and last office note from psychiatry or psychologist visit

- Current diagnosis information and information on any treatment you’ve already received
- Your referring physician's information including name, address, and phone number
- Your primary care physician’s information (if it’s different from your referring physician or specialist's information) including name, address, and phone number
- The name, address, and phone number of any specialist you’ve seen (if that specialist is different from either your primary care physician or referring physician)
- Your health insurance information including name on the policy, employer of policy holder, policy number, birth date of policy holder, insurance subscriber ID and your insurance company’s customer service/provider inquiry phone number
- Your Social Security number and date of birth
